Accomplishments 2011

Activities of Daily Living Assistance

  • 20 residents were assisted will bill payment, reading mail, and completing paperwork
  • 22 were assisted with monthly shopping for groceries & necessities
  • 10 referred to Adult Day Care and Home Health
  • In July, through the United Way Day of Caring, 50 participants received personal hygiene items, toilet paper, and paper towels delivered to their homes by volunteers. 25 participants received a meal from Smiths Grove Methodist Church also with the Day of Caring.
  • The “Help a Senior” Project through the United Way Day of Caring, received “Most Impactful Project” award during the ceremony at the close of the day.

Case Management

  • 53 received ongoing case management
  • 18 received Heating assistance through the LIHEAP Program
  • 53 seniors received Christmas Presents through the Be a Santa to a Senior Program with Home Instead

Congregate Meals

  • 27 attended monthly potlucks and outings
  • 27 attended the Annual Senior Christmas Dinner

Individual Meal Services

  • 35 receive meals daily and/or weekly through the MEALS program and Meals on Wheels through Community Action
  • 32 receive commodities monthly through the Feeding America’s Second Harvest Food Bank
  • 10 received Thanksgiving Boxes through the FOP lodge
  • 19 received Christmas Boxes through the FOP lodge

Life Skills Classes

  • 50 attended monthly classes focusing on fraud, ID theft, and disaster preparedness

Nutrition Classes

  • 50 attended monthly classes by Ms Sandhya focusing on diet, pain management ,diabetes, diseases, arthritis and blood pressure

Healthcare Referrals

  • 53 were referred to Wal-Mart, Barren River District Health Dept, and the WKU Mobile Unit for flu shots.
  • 25 were referred to Amedisys Home Health Care and Home Instead Senior Care

Medical Referrals

  • 53 were referred to the WKU Mobile Unit through the monthly news letter

Wellness Activities

  • 15-20 Participate in Bi-weekly Walking Club
  • 25 Participate in Monthly Health Education classes and blood pressure checks
  • 45 Participate in weekly social activities
